Firewall stuck in maintenance mode after a factory reset with error reason: System startup error

Symptom
After doing a factory reset, firewall goes into maintenance mode again instead of booting into the normal PAN OS.
Entry Reason: System startup error.
Click Continue to go to the Main Menu.
Navigate to Disk Image > Advanced Options
At the password prompt, enter MA1NT and press Enter.
Go to "Status" and note that the status of the partition (sysroot0, sysroot1, and maint) is EMPTY.
Environment
PANOS-10.2.7-hx
Cause
Firewall may have been in the process of formatting of the disk when it was rebooted causing other partitions to be erased.
Resolution
Maintenance mode menu > Disk Image > Advanced Options

Set sysroot0 as below, select panos then click bootstrap
Once the bootstrap is done, you should get this message:
Hit ESC.
Do the same with sysroot1 as below.
Click Bootstrap.
Once the bootstrap is done, hit ESC to return to menu.
Prepare the maint partition as below, click Bootstrap.
Once it is done, the expected screen is as below.
Go to "Status", all are ready.
Hit ESC, select sysroot0 and click Boot.
You will see the screen below, click Reboot.
Additional Information
Important: When the issue happens (first time it goes into maintenance mode after factory reset), any subsequent reboot without performing image reinstall as indicated in the above steps can result in device booting to BIOS prompt with error "Default Boot Device Missing or Boot Failed" and at this stage the device is not recoverable and needs RMA.
